The best intake form is not the longest one. It collects what you will actually use, explains what the client needs to know and does not make her answer the same thing forever.

Useful questions can include prior spray tan experience, desired depth, upcoming event timing, prep status, relevant products used on the skin and any sensitivities or concerns the client wants the artist to know about. Keep health-related questions limited to what is genuinely relevant to your service and local requirements.
A waiver acknowledgement, a service intake and permission to receive recurring marketing or automated texts are not the same thing. Keep choices clear and explicit. If a client does not opt into texts, use another eligible channel for the first contact instead of texting her just to ask for texting permission.
Short sections, plain language and conditional questions make a form much easier to finish. If a question does not change the service, the safety conversation or the client record, consider removing it.
Tan Concierge connects the intake to the client profile and can adapt the experience by service or occasion, so the form becomes usable client memory instead of a forgotten attachment.
